  • FIELD SERVICE REQUEST

  • NOTICE - 
    This lighting control system may include a factory installed startup visit. 
    It is important to follow these instructions very carefully. Failure to follow these instructions can cost you time and money. 
     
    RECEIPT OF THIS STARTUP REQUEST FORM IS REQUIRED TO SCHEDULE A STARTUP VISIT. BE SURE TO CLICK SUBMIT WHEN FINISHED.

    PURPOSE OF THE STARTUP VISIT - 
    1. To verify proper installation of the lighting control system. 
    2. To verify proper operation of the system. 
    3. To calibrate the system and perform software programming and scheduling. 
    4. To provide orientation and training for the end user.
     
    SCHEULING THE STARTUP VISIT - 
    1. Make sure all preparations listed above have been or will be completed by the date of the startup visit. 
    2. Determine the preferred date for the startup visit. (Please note required lead time) 
    3. Complete the all of the questions in this request form and click submit.

  • THE FOLLOWING QUESTIONS MUST BE ANSWERED.  FAILURE TO ANSWER EACH QUESTION FULLY COULD DELAY THE PROJECT AND COST YOU TIME AND MONEY.

  • 1) All lighting control system equipment is powered on and functioning in accordance to device health measures as shown in the accompanying documentation.

  • 2) All lighting control system equipment, as shown on the bill of materials, has been received and installed in accordance to national and local electric codes.

  • 3) All gateways have been installed?

  • 4) All wireless switches have been installed?

  • - All are installed
    - Some still remain to be installed
    - This project does not have any

  • 5) All energy managers and PoE switches provided by Enlighted are installed within Enlighted enclosures with non-switched receptacles?

  • - All are installed
    - Some still remain to be installed
    - This project does not have any

  • 6) All PLR (Plug-loads) have been installed?

  • - All are installed
    - Some still remain to be installed
    - This project does not have any

  • 7) Will system training be necessary for the end customer? If yes, arrangements must be made for representatives of the end user to be on site for orientation and training of the system (if required). There are no allowances made for return training visits, unless specified in the bill of materials.

  • 8) Are there any security measures required to access the site and(or) lighting control equipment? If yes, please explain any special security measures (surrender of IDs, security clearances, safety training, escorts, etc.) required to access the site and/or lighting control equipment.

  • 9) Are there any PPE requirements for this site?

  • 10) Please provide the utility rate, in cents. This is used to show the energy usage in the dashboard of the energy manager. If left blank, the amount of .12 will be used.

  • 11) ELECTRICIAN MUST BE PRESENT -

    An electrician familiar with the installation must be present while Enlighted's field tech is onsite. Ladders and(or) lifting equipment shall also be readily available.

  • 12) AS-BUILT DOCUMENTATION NOTICE -

    All as-built documentation is complete, accurate, and has been submitted at least one week prior to the startup date. 
    (FAILURE TO PROVIDE THIS INFORMATION MAY RESULT IN FORFEITURE OF YOUR SCHEDULED DATE.)

    The as-built documentation must include INDIVIDUAL UPLOADS.

  • 13) ADDITIONAL CHARGES - 

    If extra trips or extra time is required to complete the system due to lack of preparation or installation problems, additional charges to the contractor will result. This extra work will require a purchase order.

  • 14) CANCELLATION - 

    In the event you cancel your confirmed date without reasonable notice, you may be charged to reschedule.
    $600.00 - If notice is less than 3 business days from startup date
    $300.00 - If notice is 4 to 7 business days from startup date
