  • Have you ever lost or given away a pet?*
  • Are your present pets up-to-date on their annual vaccines?*
  • Are your present pets spayed or neutered?*
  • Do you understand that you will be responsible for your new kitten's required medical care, including, but not limited to, spay/neuter (if not done prior to adoption), vaccines, any testing?*
  • Do you agree to have your new cat(s) spayed/neutered, at your own expense, by 5 months of age. (Within 3 mos for 8 week old kitten.)*

  • If your cat displays behavioral problems (such as poor litter box habits, inappropriate scratching etc.) how would you go about correcting the behavior?*
  • What type of solution would you be willing to try if housebreaking accidents continue after the first week (check all that apply)?*
  • Is your entire immediate family/housemates in agreement with the decision to bring a new pet into your home?*
  • Are you prepared to commit to a pet for 15 - 20 years (average life span)?*
