This consultation card is used to evaluate your individual skin care needs. We will maintain the confidentiality of this information, and will disclose this information only to 1)Our Staff & 2) Our product supplier and manufacturer (if the result of a reaction). We will not provide the information to anyone else, except as required by law, and we will not sell this information to anyone.

RESERVATIONS

Services are available by appointment only. We recommend scheduling your appointment as far in advance as possible to ensure availability. A credit card on file is required for all appointments.

CANCELLATION

We value your business and appreciate your cooperation with our cancellation policy. We require that you advise us at last 24-hours in advance if you need to to change or cancel an appointment we have held for you. We may charge up to the full amount of service for a cancellation within 24 hours of your appointment. If you fail to appear for your appointment without attempting to cancel your appointment beforehand, the full amount of the service will be charged. Appointments booked for the same day are considered confirmed and cannot be cancelled, to adhere to our 24-hour cancellation policy.

LATE ARRIVALS

Please understand that we allot a specific amount of time for each service. All appointments have been designed to allow the appropriate time for service. Your late arrival may limit our ability to offer the fullest experience possible. Please understand that late arrivals will not receive an extension of scheduled treatments. You will be responsible for the full service amount.

ETIQUETTE

Please put your cell phone on vibrate while visiting our Studio, so as to not disturb other guests during their treatment. To allow full relaxation at our space, we ask that you do not bring children, guests or pets along to your appointment.

PAYMENT

We accept most forms of payments (Cash, Credit/Debit Card, and Venmo), please know that we do prefer cash or Venmo.

By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
