Massage Policies:
- Your scheduled session is set aside for you. If you are late for your massage session, time will be deducted in order me to remain on time for other clients the rest of the day. Please respect my time and yours.
- 24 hour cancellation notice is required to avoid being charged for your session. Emergency cancellations are determined at the practitioner’s discretion. "No-call, no-show" appointments are subject to 100% charge.
- Be clean, having bathed the same day as the treatment if possible.
- Be present - turn off your cell phone for optimal relaxation. Any clients who arrive under the influence of alcohol or drugs will be required to reschedule and pay a 50% fee for the missed original appointment.
- Prior to your massage, you will have a consultation with your therapist to discuss your session. Should the session require, after your therapist has left the room, you may disrobe to your comfort level before getting on the massage table.
- You will be modestly draped throughout the massage and at no time will genitalia or breast tissue be exposed.
- I understand that my therapeutic massage therapist or I may end the session at any time for any reason
- Client services and chart information are confidential. Written authorization is required from the client to release any information.
Client Agreement:
- I understand that the massage I receive is provided for the basic purpose of relaxation and relief of muscular tension.
- If I experience any pain or discomfort during a massage therapy session, I will immediately inform the practitioner so that the pressure and/or strokes may be adjusted to my level of comfort.
- I understand that massage therapy should not be constructed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician, chiropractor, or other qualified medical specialist for any mental or physical ailment that I am aware of.
- I understand that massage therapists are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the massage session(s) given should be constructed as such.
- Because massage therapy is contraindicated under certain medical conditions, I affirm that I have stated all of my known medical conditions, and answered all questions honestly.
- I agree to keep my massage therapist updated as to any changes in my medical information, and understand that there shall be no liability on the part of the Massage Therapist (Jane Neldon, LMT), or on the part of Commonwealth Massage Therapy, LLC should I forget or fail to do so.
- I understand that inappropriate behavior will not be tolerated and may be prosecuted to the full extent of the law. Massage sessions are not of a sexual nature. Sexual or provocative behavior and/or sexually suggestive remarks or advances during a massage session are always unethical and inappropriate. Such behavior will result in immediate termination of the session with payment due in full. Future appointments will be denied.
