PBGH PROMOnc Test Site Staff Questionnaire
Thank you for choosing to complete this survey online and sharing your perspective as a staff member involved in PROMOnc project at one of our 23 participating test sites. Your responses will be anonymously submitted to PROMOnc Project Team. Results are collected and aggregated for research purposes in order to assess feasibility and understand the burden and benefits for oncology care providers on implementing PROMOnc surveys.

What types of staff performed patient engagement and administration of survey? List all roles and describe their responsibilities (ie. MD introduces patient to PROMOnc survey, Front desk Administrative Assistant checks in patient and provides iPad for survey completion).
