I agree to not let anyone into the gym. If I do let someone into the gym Im responsible for any damage that person does to the gym. I agree to pay for any supplements or drinks I take from the pro shop. Please pay with cash or Venmo at that time. I agree to put all equipment away that I use and wipe it down after each use or I’ll be asked to not come back to the gym. I agree to not disrespect anyone at the gym or I’ll be asked to leave. I agree to shut off all lights and close all doors if I’m the last one out of gym. If I’m training with a trainer, I agree to leave when my personal trainer leaves the gym. I know I cannot be at the gym unless my trainer is present or I buy a day pass via VENMO or purchase a keycard membership at www.614GYM.com If you have any questions or concerns please text James at 614-570-4328

I hereby release and agree to hold The Warehouse Gym & Fitness LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the gym, or that may otherwise arise in any way in connection with any services received from The Warehouse Gym & Fitness LLC. I understand that this release discharges The Warehouse Gym & Fitness LLC from any liability or claim that I, my heirs, or any personal representatives may have against the gym with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from The Warehouse Gym & Fitness LLC. This liability waiver and release extends to the gym together with all owners, partners, and employees. I understand Gym is under 24 hr surveillance and pictures and video may be taken for TWGF for promotional uses or legal action. I agree not to slander or injure the business reputation or goodwill of the The Warehouse Gym & Fitness also known as TWGF or the TWGF Affiliates through any contact with customers, vendors, suppliers, personal trainers or agents of the TWGF or the TWGF Affiliates, or in any other way. I agree not to recruit members or current clients of TWGF to other trainers. I understand and agree no refunds will be given once money is paid to TWGF or personal trainers of TWGF.
