Under the terms of the Association for the Scientific Study of Anomolous Phenomena (ASSAP), National Register of Professional Investigators (NRPI), Code of Ethics, we are required to obtain the following information to determine the type of case we are dealing with. During any interview stage, along with all relevant details regarding your case, we will ask questions regarding, but not limited to, your current mental health status, your use of both prescription and recreational drugs and alcohol use. Your answers will be discussed further at the telephone interview stage (if applicable) to assess whether we are legally obliged to continue with the case or not. If you agree to giving us this information and are over the age of 18, please confirm below. All information is held in the strictest of confidence and will not be divulged to any third parties without your written consent.
