Notice: The Durant City Utilities Authority (DCUA) utilizes the services of Perdue Brandon Fielder Collins & Mott LLP, a consumer credit reporting agency as defined under the terms of the Fair Credit Reporting Act. As a subscriber, the DCUA is an information source providing data concerning delinquent accounts to the agency.
By signing below, applicant certifies that all information contained in this application is accurate and complete, and authorizes employees of the Durant City Utilities Authority (DCUA) to contact any and all references given. Applicant further authorizes the DCUA to use any additional consumer or business reporting services at its disposal in order to reach an informed credit decision. Applicant further certifies that the DCUA’s credit terms of net amount due by the due date of the month following billing are fully understood and agreed to, and proper payment within these terms in consideration of extended credit will be made. Unless the applicant disputes the validity of their debt within thirty (30) days of receipt by writing, by phone, or in person, the DCUA will assume the debt is valid. Applicant understands that the DCUA reserves the right to refuse service to anyone for any reason and that in the event that a utility account is established, this account may be terminated at any time should the account become past due or delinquent. Applicant also understands that with the extension of service by the DCUA, applicant assumes all responsibility for any amounts due to the DCUA for services provided, and any collection fees, penalties, legal fees or interest charges incurred for the collection or recovery of delinquent amounts owed to the DCUA. If the trash container is removed from the service address, the applicant’s account deposit will be forfeited to cover its replacement cost. Applicant further understands that the DCUA reports delinquent account information to Perdue Brandon Fielder Collins & Mott LLP. This application becomes a contract upon the establishment of service.
