Use this form to schedule an appointment for in-person support with the initial claim form.
In-person support is intended for staff members who do not have access to a device or internet at home, and those who need language assistance.
Do not make an appointment:
- If you have forgotten your iVisions password. Please submit an IT ticket to retreive your password at https://support.rsd7.net/portal
- If you need to update your address. Please update that information in iVisions.
In-person Support Dates: Monday, June 1, 2020 - Thursday, June 4, 2020
Times: 9:00 a.m. - 2:00 p.m.
Location: Transportation Department Building C
In order to maintain social distancing, we ask that you follow these guidelines if you need to come to Transportation Building C for support:
- If possible come alone, or limit the number of individuals who must come with you;
- Wear disposable or reusable (cloth) face coverings;
- Wear gloves;
- Follow all directions of the volunteers; and
- Maintain proper social distancing at all times.
Prior to your appointment, be prepared with the following information:
- Access to your iVisions Web Portal (username and password)
- Social Security Number.
- Start date with RSD. If your employment with RSD started before November 1, 2018, an estimated date will be adequate.
- Work history for the last 18 months, including dates of employment, your employers' business names, addresses and phone numbers.
- Estimated salary and total income from each employer.
- If you are not a citizen of the United States, you will need your Alien Registration Number and documentation.
- If you are a veteran, you will need your DD-214.
- Gross pay - entering an estimate will be fine.
