We recommend seeking professional grooming services for breeds such as poodles and poodle mixes (doodles), bichons, etc, due to the complexity and extensive grooming needs these breeds tend to need.

I, the undersigned, hereby certify that I am the owner or duly authorized agent for the owner of the aforementioned animal. I hereby give GAVH full and complete authority to bathe and perform the procedures listed on this form. GAVH reserves the right to treat your pet in case of an emergency. A set amount of time will be appropriated for prepping baths; excessively matted pets are subject to additional fee and depending on the extent of the matting, we may recommend a professional grooming service.
