In response to the COVID-19 crisis, SUNY Optometry converted all classes to online learning during the spring 2020 semester. The federal Coronavirus Aid, Relief, and Economic Security (CARES) Act authorizes colleges to award emergency financial aid grants to students for their expenses related to the disruption of campus operations due to coronavirus, such as course materials, technology, health care, and child-care expenses. This form allows students to request a federal CARES Emergency Grant.
Complete the request below if you, the student, have already incurred expenses that are related to the disruption of campus operations due to coronavirus. In order for an incurred expense to be eligible, it must be related to the disruption of campus operations. Eligible expenses include course materials, technology and computer, health care, dependent care, transportation, and other. Grant amounts will depend on the number of requests received by the College. Preference will be given to students with high financial need. Students must be Title IV eligible which means they qualify for federal aid programs (like Stafford loans).

- For each expense, write brief description, describe how it is related to the disruption of campus operations due to coronavirus, and specify the amount. Financial Aid may request additional information, documentation or receipts to resolve any conflicting information, clarify a request or to justify an extraordinary expense amount.
