    IF YOU NEED TO FILE FOR MULTIPLE YEARS, PLEASE SUBMIT A FORM FOR EACH YEAR. 

  • How did you find out about us? (Please be specifics such as the person’s name, the event you attended, the name of the Facebook group, Google Ads, FaceBook Ad, TikTok Ads, The news, etc.*
  • Are you or your spouse a non resident alien, or a citizen of another country?*
  • What type of Visa are you on?*
  • Did you switch from 1 visa to another visa in 2025? This typically happens when people go from a student visa to a work visa.*
  • Do you have form I-94?*
  • If you're a returning client, has your email address changed?*
  • If you're a returning client, has your phone number changed?*
  • If you're returning, do you need to provide a new address for your tax return?*
  • If you're a returning client, do you need to remove or add a dependent from last year?*
  • Are you male or female?*
  • Are you a college student, veteran, disabled, or age 65 and older?*
  • Is your driver's license and where you are registered to vote different than the state listed on your W2? If so, you are domiciled in that state, & will have to file 2 state tax returns (1 non resident, and 1 resident).*
  • Did you work in 1 state, and live in another?*
  • Do you have income from more than 1 state?*
  • Did you work overtime at your W2 job in 2025? IF YOU WORKED OVERTIME, IF THERE ISN'T INFORMATION IN BOX 14 OF YOUR W2, WE WILL NEED A COPY OF YOUR LAST PAYCHECK STUB. If you qualify for this deduction, we will need to complete an addtional form which is billed at $50. If you don't provide the last paycheck stub, we cannot take the deduction.*
  • Did you earn tips at your W2 job, or as an self employed person? If you qualify for this deduction, we will need to complete an addtional form which is billed at $50.*
  • If you receive tips, does your employer mandate gratuity? Said another way, do customer receipts have tips autotmatically added? If your answer is NO, and you qualify for this deduction, we will need to complete an addtional form which is billed at $50.*
  • Did you purchase a new 2024, 2025, or 2026 car in 2025?*
  • Do you have biological children under the age of 11 that you are claiming on your tax return?*
  • Do you want our office to facilitate eligibility, prepare, and file the required IRS Form 4547 to elect the TRUMP ACCOUNT? This is a FREE $1,000 U.S. Treasury seed (if your child was born in 2025), or a FREE $250 deposit (from the Michael & Susan Dell Foundation) if your child/children are among the 1st 25 million that live in a zipcode where the median income of $150,000. THE COST FOR THIS SERVICE IS $50.*
  • Do you have children that live in your home that you did not birth/father, that you take care of financially, that you will be claiming on your tax return? YOU MUST HAVE PROOF THAT THEY LIVED IN YOUR HOME. Proof according to the IRS is daycare/school records , medical records, or social service records.*
  • Did you, or your spouse birth a child in 2025?*
  • Do you have biological children UNDER THE AGE OF 17 (or under the age of 24 if in college), that live in your home that you financially take care of (that you claim as a dependent on your taxes)?*
  • Did anyone else live in the home that provides financial support for your child/dependent(s)?*
  • Did your dependent child/children work a job?*
  • The cost to file a dependent's tax return is $100 FOR W2 INCOME only. Education credits, 1099 income, etc. are billed additionally. For 2025, if your dependent earned less than $15,750 as an employee, they are not required to pay FEDERAL TAXES, but if you live in a state that taxes income, they may have a state filing requirement. State returns are based on Federal AGI, so we need to file a Federal and state tax return (included in the $95) DO YOU WANT US TO FILE THE DEPENDENT TAX RETURN?*
  • Did you pay DAYCARE/CHILD CARE/SUMMER CAMP/ADULT CARE EXPENSES for a child/children or adult dependent that you claim ?*
  • Did you have a child between the ages of 18 and 24 that was in COLLEGE, OR TRADE SCHOOL, that lived in your home as your dependent?*
  • If you have a child in college, do you have the 1098-T from the college that YOUR CHILD attended? IF NOT, YOU MAY NOT BE ABLE TO TAKE THIS DEDUCTION. Call the college, or trade school to get the 1098-T. Your child can also look in their financial aid records, or school email for this document.*
  • Have you ever been disallowed the Earned Income Credit/The school credit (American Opportunity Credit, or the Child Tax Credit?*
  • ARE YOU MARRIED OR SINGLE?*
  • If you are married, do you file with your spouse, or do you file separately from your spouse? Please note, even if you file separately we will still need the spouses name, and social in order to efile. If your spouse doesn't want to give their info, the tax return will have to be mailed.*

  • Did you get your health insurance from the Marketplace healthcare.gov? If you did, please make sure to provide us with the 1095-A document. If you do not provide this document, the IRS will reject your return, and it will cost you an additional $19.95 for us to resumbit the return after you provide the information on the form.*
  • Did you have a HSA at your job? (Health Savings Account).*
  • What type of HSA plan do you have?*
  • Did you have withdrawals from your HSA account?*
  • Were all of your HSA withdrawals used for medical reasons? If audited, you will need to prove this with receipts.*
  • Did you trade crypto? Trade means buying and SELLING, not just buying.*
  • Did you trade stocks/options on a platform like Forex, Robinhood, etc.? If so, please upload your 1099-B form.*
  • Did the IRS give you or any of your dependents an IP PIN? If the IRS gave you an IP PIN, and you do not provide it at the time of filing, there will be a fee of $19.95 for us to resubmit your tax return for electronic filing.*
  • Did you work a job and receive a W2?*
  • Do you (or did you), do things on the side to earn additional income such as: Lyft, Uber, Doordash, Amazon flex, Sell Avon, etc. to earn additional income?*
  • Did you receive a 1099-NEC for work you did as an independent contractor?*
  • Did you have a business, side hustle, or side gig where you collected CASH? ** Please note, BANK DEPOSITS, ZELLE, CASHAPP, APPLE PAY, VENMO, ETC. ARE CONSIDERED THE SAME AS CASH. The question is NOT asking about rentals. Rentals are addressed further down.*
  • If you have a business, sole proprietorship, or side gig, what type of entity did you create? If you DID NOT create an entity, but you did something on the side to generate additional income, please select no entity just a sole proprietorship.*
  • Did you win any money gambling on the lottery, the boat, horse racing, etc.? If yes, you need to provide the W2-G forms.*
  • Do you pay a mortgage on the home you live in (are you a homeowner)? If so, PLEASE REMEMBER TO PROVIDE YOUR ACTUAL PROPERTY TAX BILL (not just the 1098), & your mortgage interest statement (if you're still paying a mortgage).*
  • Did you sale a home in the tax year? If so, we need the tax form 1099-S that was provided to you from the title company.*
  • Do you own rental property that was rented out during the tax year? ** Please note, part of the year rented is considered yes.*

  • Do you have individual retirement accounts, investment accounts, or mutual funds OUTSIDE OF YOUR JOB (NOT YOUR JOB'S 401K)?*
  • DO YOU PAY THE MORTGAGE/RENT, AND BUY THE FOOD FOR YOUR ELDERLY PARENTS? You must be able to prove this if you're ever audited.*
  • Did you have medical expenses that were more than 7.5% of your income? Medical expenses DO NOT include premiums paid for healthcare. Qualified expenses are copays, deductibles, glasses, xrays, dental fillings, etc.
  • Did you donate cash to a non profit, church, or other charity? If so, how much did you donate. You must have a letter from the organization to prove this donation, or copies of zelle payments, checks written, etc. This DOES NOT include money that you give to the homeless, or at the grocery store cash register*

  • Did you purchase a new vehicle, or boat, that you paid state sales taxes on? If so, how much sales tax did you pay? Please note, you must have WRITTEN PROOF!
  • Were YOU in college or trade school during the tax year?*
  • Do you have the 1098-T from the college that YOU attended?
  • Aside from health insurance premiums withheld from your paycheck, what healthcare expenses did you have? This includes medically necessary dental (not veneers), glasses, prescription copays, medical equipment, etc.*
  • Do you live in Indiana, Wisconsin, Michigan, or Maine?*
