Use this form if a parent/guardian requires an accessible version of their student’s assessment score report. Once submitted, the request will be processed as soon as possible, but may take up to 30 days. The accessible score report will be sent to the school district, to provide to the parent/guardian. The SDE cannot release a student assessment score report directly to a parent/guardian.
