Please submit this form to recieve your certificate of completion and hard hat stickers. The certificiate of completion will be sent electronically to the email given below and the hard hat stickers will be mailed to the address below. Materials will be sent out after June 22, 2020 once the stand down has been completed.
If your company held more than one stand down at mulitple job sites you will need to submit this form for each stand down/job site. Please do not combine forms. We ask that you answer all the questions listed below. If you have pictures of your Stand Down event and would like to send, please email them to email@example.com.
