1. I am fully aware and acknowledge that exercise or use of a golf simulator presents inherent dangers and could lead to injury, disability, illness or death.  I am also aware that these types of activities should not be engaged in without authorization from a medical professional.  I agree that any recommendation for changes in diet including the use of food supplements, weight reduction, or body-building enhancement products are entirely my responsibility and that I should consult a medical professional prior to undergoing any dietary or food supplement changes.  I agree that by voluntarily participating in exercise programs, use of the golf simulator or training activities that I do so entirely at my own risk.  I further agree that I shall strictly adhere to all CDC, State, and local government current and future guidelines regarding COVID-19 and other diseases or illnesses of any kind.  

2. I expressly agree, on behalf of myself, my heirs and assigns, to hold harmless, waive,release, defend, and indemnify Body In Rhythm Training LLC, its owner(s), Member(s), agents, employees, representatives, successors, and affiliates from any and all claims or causes of action for any personal injury, illness (including, but not limited to, any viruses of any kind), damage, losses of any kind, or potential claims that may arise from my engagement in any exercise programs,use of the golf simulator or training activities of any kind or use of any equipment.  I expressly understand that I am releasing, discharging, and waiving any claims or actions that I may have presently or that may arise in the future caused in whole or in part by the acts, omissions, and/or negligence of Body In Rhythm Training LLC, its owner(s), Member(s), agents, employees, representatives, successors, and affiliates.

3. I agree to pay for damage to any equipment owned by Body In Rhythm Training LLC that is caused by me or my guests careless, intentional and negligent acts.

I am at least 18 years old and am competent to execute this Waiver and Release.  I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a release of liability.  I am signing this Waiver and Release voluntarily and agree that no oral representations, statements or inducements apart from this written agreement have been made to me.
