Honesty is the best policy. Please be honest with staff about the pet you are wishing to release. We recognize it is not easy to be up front about the reasons for surrender and can be an embarrassing experience. Honesty about the animal’s background is in the animal’s best interest so we can better pinpoint history and behavior. Honesty allows staff to be able to help much more efficiently.

Owner Release Agreement
1. I understand that upon signing this release form and surrendering the animals listed to Lewisville Animal Services, I am immediately relinquishing all interest, rights, or claims to them. I further understand that the City of Lewisville, including any of its employees, will not be obligated to me in any way regarding the animals or their final disposition.
2. I understand that the animal(s) may be offered for adoption, placed with a humane organization, or humanely euthanized at any time after I have surrendered them to Lewisville Animal Services. However, I further understand that the City of Lewisville does not promise or otherwise assure that the animal(s) will be kept at Lewisville Animal Shelter for any minimum period of time, or that they will be offered for adoption, or that they will be placed with a humane organization.
3. I understand that if I want to get animal(s) back, I will be required to comply with all City of Lewisville adoption policies and requirements including completing the applicable adoption forms, paying the applicable fees, obtaining required vaccinations and registrations, and having the animal(s) spayed/neutered. All adoptions are made on a first-come-first-serve basis.
4. By my signature below, I certify that I AM the lawful owner of the animal(s) described on this form and that the information contained herein is true to the best of my knowledge. I have read the terms and conditions of this release and hereby surrender the stray animal(s) listed on this form to the City of Lewisville Animal Services.
