1. Explanation of the Session
Your training sessions will involve incremental changes in intensity to create physiological adaptations, monitored by your trainer.
The session may consist of any combination of mobility, dynamic stretching, strength training, and power training exercises.
There is the potential that you will experience soreness or fatigue in the musculature during or after exercise
2. Risks and discomforts of the exercise session
There is a potential that your cardiovascular or respiratory system could experience unexpected changes due to participation in aforementioned exercises.
These changes include abnormalities of blood pressure, fainting, disorder of heartbeat, and in rare instances, heart attack, stroke or death.
If you notice any unfamiliar signs or symptoms to your breathing or heart rate, immediately bring to the attention of the trainer so we can make the appropriate changes necessary.
If you experience any discomfort in the musculoskeletal system please make it known immediately to make adjustments to the exercise program.
3. Confidentiality
The information gathered during our sessions based on observation or during conversation is treated as privileged and confidential.
4. Inquiries
You may refuse to participate now or stop at any time during the exercise session. It is your decision. Before signing this form, please feel free to ask any questions regarding any aspect of this program that may be unclear to you.
