Quarantine  Hosting Consent Form and Protocol

Please read carefully each step and acknowledge your understanding for each risk factor and measure presented.Your consent and signature at the end is mandatory, since filling this out will determine your eligibility to be contacted by our team in order to host ILAC students as soon as they arrive into the country.
