Book Loan Contract

The Center for Teacher Education will provide free loan of the textbook(s) selected below on a first come, first serve basis as textbooks are available.  Spring Intersession return date is Monday, January 30, 2023 and the Spring semester return date is Monday June 5, 2023.  Prior to the due date, an email will be sent notifying you of available return dates, times, and procedures.  Please regularly check the email address you list in this form to ensure you remain updated about our Book Loan Program.

Which textbook(s) are you requesting to borrow?

Limited quantity available.  Textbooks will be distributed based on the order of form responses.  Students may request textbooks for up to 2 courses.

Please double-check your textbook information through the Don Bookstore, your class syllabus, or your Canvas course prior to making your selection.

Book Loan Policy

Read this policy in its entirety and sign below to acknowledge your agreement:

By checking out this book, I understand that:

• The textbook I am requesting must be for a Santa Ana College course that I am currently enrolled in.
• I must return the textbook on or before the last day of the semester that I checked it out.
• If the textbook is 1 week late, then a HOLD may be placed on my academic records.
• If the textbook is 2 weeks late, then I may be charged a $20 late fee (in the form of Don Bookstore gift card).
• If the textbook is late by 1 month or more, I may be charged the retail price of the textbook (if not returned).
• If I decide to officially/unofficially withdraw from the class I must return the book to the CFTE immediately.
• If I drop or don't pass the class and would like to re-borrow the book, I must first return the book to the CFTE.
• I am responsible for paying the FULL VALUE of the book as listed in the Santa Ana College Don Bookstore if the book is lost, stolen, or severely damaged.
• If I fail to comply with the above guidelines, I can lose my privilege or borrowing books in the future.
