  • This Patient Handbook is designed to provide you with important information regarding your health services, office policies and fee structure.

  • What is Integrative, Functional Medicine?

  • Integrative Medicine is defined as the practice of medicine that connects the patient and provider through a whole person approach considering the unique underlying factors and risks that contribute to health and wellbeing, including lifestyle and environment. Functional medicine aims to uncover and address the root cause of illness with an evidence informed approach. Dr. Pearlman defines her Functional Medical model by the pillars of 5Ps: personalized, preventative, precision, proactive partnership- based medicine. 

  • Working closely with each patient, a customized care plan is designed to address unique genetic predispositions, hormonal and metabolic profiles, lifestyle factors and environmental exposures. Integrative and Functional Medical services are presently not insured under OHIP. In providing these services, Dr. Pearlman and our team will compliment and NOT REPLACE your primary care physician.

  • Customized Health and Lifestyle Plan

  • A Customized Health and Lifestyle Plan will be provided to you following a comprehensive consultation and functional medical evaluation. This plan will address a range of factors (your health print) such as hormonal, metabolic, immunologic, genetic, nutritional, lifestyle, mental health, and environmental determinants of your health and aging. The plan is refined throughout the program outlining the key nutrients, health supplements, hormonal therapy (where appropriate), and other forms of medical management as appropriate.

  • Multi-Disciplinary Health Team

  • You will be working with our multi-disciplinary integrative health team including nurses, nutritionist, health coaches, fitness providers, etc., who will collaborate and communicate regarding your care to optimize your results and outcomes.

  • Virtual Health Platform and Remote Care

  • As part of our programs, you will have access to medical care through our Virtual Health Platform. Our Virtual Care is delivered through a secure medical system that ensures your privacy while integrating seamlessly into your medical record.     Your consent to participate in Virtual Care is required. Please review and complete this consent form prior to your appointment.   • Extended Care Services

  • We offer a range of concierge services to facilitate your care and ensure your experience is seamless helping you to get the most of your program. Extended Care Services include; healthcare navigation, health advisory, prescription management, proactive referral management, virtual care, remote services, second opinion and non-emergent care available. Please note that we DO NOT OFFER emergency medical services

  • Prescription Management

  • Prescriptions will be written at the time of your medical assessment to last until your next scheduled appointment (typically 3 months). Should you require a prescription repeat or dose change, we will gladly arrange this with your pharmacy. We offer extended medication management for any specialized prescriptions, such as compounded hormones, to help ensure quality and to provide customer service and convenience.

  • Appointment Scheduling

  • We will do our best to accommodate your scheduling needs. We recommend booking appointments well in advance to best match your needs. While we will remind you about your next appointment, it is up to you to ensure that you schedule and keep your planned quarterly visits. We do not commit to any fixed number of appointments over the course of the year, nor do we limit appointments in the case that additional care is required or new concerns arise.

  • Cancellation Policy

  • We have a strict 24-hour cancellation policy. Please notify us immediately should you be unable to keep a scheduled visit so that we may offer your appointment to another patient requiring care. A charge of $150 is applied for appointments cancelled within 24-hours.

  • What to Provide on Your First Visit

  • Please forward to our office or bring to your visit copies of any relevant medical results such as your latest mammogram, pap test and bone density results and any recent blood work. Please bring a detailed record of any prescription medications, supplements and hormone therapies that you are currently using. Please be sure to advise us of any drug allergies or past medical history. You will be asked to present a valid OHIP card at each visit and provide updated contact information.

  • COVID-19 Office Protocols and Screening

  • Due to the COVID-19 global pandemic, we have enhanced our measures of infection control and screening. Before any in-clinic appointment you will be contacted by our team and asked to complete COVID-19 screening that can be reviewed here. We have careful protocols that are required to ensure your health and security at the clinic. You can review these policies here.

  • Clinical Evaluation, Specialized Testing and Diagnostic Imaging

  • We offer a range of clinical testing including evaluating your hormones, metabolites, nutrients, immune markers, genetics and disease markers. Some of our specialized test can be done from the comfort of your home as a self-administered test. Other tests require an office visit or acquisition at a nearby laboratory. Diagnostic imaging is arranged at an appropriate Centre such as a nearby diagnostic imaging lab or hospital. Several of our specialty tests are not be insured by OHIP and if not included in your health program may involve additional costs.

     

  • Program Design and Care Plan

  • Your health program involves a comprehensive onboarding process during which a detailed health history, intake assessment, examination, clinical and specialized testing will be performed. Screening and diagnostic imaging will be arranged as appropriate. A customized health and lifestyle plan is generated and refined over the course of your program.

    Quarterly follow up assessments allow regular updates to stay ahead of new issues. We ask that you advise our team of any important changes in your health status that may occur at or between this quareterly meetings.

    Our integrative health team will be working together to co-ordinate your care. While your care is medically supervised, our nurses, nutritionists and health coaches each play a vital role in delivering your care and you may meet with all or some of these providers at your scheduled visits. 

     

     

  • Health Advisory and Medical Emergencies

  • If you experience a medical emergency including but not limited to chest pain, anaphylactic shock, respiratory distress, etc., please call 9-1-1 and go to your nearest Emergency Department.

    Telehealth Ontario at 1-866-797-000 offers 24-hour nurse led medical advice including guidance on COVID-19 related issues. For a non-emergent question or concern related to your care please contact our office by calling 416.644.1112. We aim to respond to health inquiries by having a team member respond within 1 business day.

  • Uninsured Services and Fees

  • We offer a blended model of care involving both conventional and complimentary or alternative medicine as part of our integrative approach. This means that there are both insured and uninsured aspects of care.

  • Fees are voluntary and relate only to those services provided to you that are not insured under OHIP.

    Fees may be paid in full or over a series of monthly installments. Outside of program fees, you may choose to pay for individual health services at the time the service is rendered. A detailed breakdown of Program Fees is contained separately. The College of Physicians and Surgeons, the CPSO, has provided information relating to Block Fees and Uninsured Services here.

    The list of uninsured services includes but is not limited to: integrative and functional medical services, prescribing of customized bioidentical hormone therapies, weight management counseling (for non-obese individuals)*, customized lifestyle planning, naturopathic, homeopathic and dietary interventions, preventative health assessments (beyond one per year), nutritional counseling, personalized supplementation, genomic profiling, extended care services (i.e. prescription management and renewal, etc, and any intervention and/or service considered to be alternative or experimental in nature. Extended and remote care and administrative services are not insured under OHIP. Included in our Program Fees are the following services. Additional fees may be incurred if further services are required.

    • Virtual Health Consults: $500 and up per session (30-minutes)
    • Case Conference: $150
    • Genomic Profiling: $1500 and up
    • Whole Genome Sequencing: $3000 and up
    • Personalized Nutrition: $1500
    • Gut and Immune Testing: $1500
    • Health Coaching: $150 per session (30min)
    • Forms and Letters: $150 and up
    • Vitamin Injections: $45 and up
    • iv Vitamin Therapy $250 and up
    • Prescription Management: $45 per service
  • Program Fees

  • We offer health programs as a basket of services and specialized tests to enable comprehensive care in a manner the optimizes the physician-patient relationship. 

    Program Fees are non-transferrable and non-refundable after 10 business days from date of payment, save for exceptional cases. Your explicit consent for Fees relating to uninsured services is required before such services can be provided. Program Fees are not an “access fee” required to receive services insured by OHIP. Dr. Pearlman offers alternate options for care involving insured medical services.

  • Payment Information

  • Payment for Program Fees may be made in cash or by debit or credit card. Personal cheques cannot be accepted. Payment can be made in full prior to the inception of care. We offer a payment plan option with your payment information stored securely for recurring payments. Please inquire about this option.

  • Disclosure Statement and Consent
