  • Feedback is valuable in ensuring that DVP-funded organizations providing services within their communities meet certain standards. You can find a list of DVP-funded organizations by clicking here.

Please complete the following form to provide your feedback.

  • Feedback from an individual who used or attempted to use services at a DVP-funded organization

  • Please note: DVP cannot provide immediate solutions to dissatisfaction with services. DVP-funded organizations operate independently and DVP does not have the authority to alter organization decisions regarding shelter stays or other service provision.

All feedback will be reviewed and a DVP team member will reach out to you if additional information is needed. DVP will notify the organization that a concern was reported. Your feedback may also be reviewed by the DVP Advisory Committee.

  • So what will happen next with your feedback?

Depending on the nature of the feedback, DVP may simply listen to concerns in order to understand in general where organizations may need support without taking any actions beyond notifying the organization that a concern was reported.

  • Feedback from an employee or volunteer at the of a DVP-funded organization that I am providing feedback about

  • DVP is not able to arbitrate employee-employer concerns. As an employee or volunteer of a DVP-funded organization, you should have received either Employee or Volunteer policies which should include information about how to handle concerns or conflict. For example, many organizations instruct workers to talk with their immediate supervisor about their concerns. If the concern is not resolved with the supervisor, addressing concerns with the executive director and/or board of directors may also be appropriate. Organizations should also provide their employees and volunteers with information specific to whistleblower situations. If you feel it may be useful to discuss your organization’s policy guidance, or other State entities such as the Department of Local Affairs, please call DVP at 303-866-3150.
