This tool will generate two forms which are referenced in the Disaster Management Act as Form 4 and Form 5.
Form 4 is required by each and every person travelling to attend a funeral. This form must be printed and presented to a police officer or magistrate for certification.
However, in order for the Magistrate of Police office to certify Form 4, they will require a copy of the death certificate for the deceased individual.
If the death certificate is not available, for example, because cultural and religious rites require burial within 24 hours, then Form 5 will be required, along with a letter from a religious leader indicating that burial must take place within a required time.
Instructions:

- Repeat the task for each person travelling in your party
- Print all the forms and present them, together with a copy of the death certificate, and/or a letter from a religious leader at your local police station or magistrate
- Keep the original forms on your person when travelling to be presented together with identification.
