Submitting this log sheet to Mach Martial Arts does not mean that the hours indicated on it will automatically be applied to the President's Volunteer Service Award Program. All volunteer hours are subject to verification.

If service hours indicated on this sheet are with or for an organization outside of Mach Martial Arts, Letter must:

1. be signed on the organization's official letter head (showing outside organization's logo, etc.)

2. include the specified number of hours

3. acknowledge that the hours are being used by Mach Martial Arts for the President's Volunteer Service Award and will not be utilized for the same purpose by the outside organization.
