Considering the current COVID-19 situation, we ask that you use extreme caution when determining whether your child should attend AWANA.
If you, your child, or a member of your family are medically considered "high risk," please understand the limitations of working with children and social distancing. We would love for your child to attend and therefore, leave the risks of COVID-19 exposure up to parents and guardians.
If you, your child, or a member of your household at any time, during the dates of AWANA, exhibts symptoms of illness (cough, fever, sore throat, or any other symptoms), we ask you keep your child home.
Further, in the event you, your child, or a member of your household has had an illness, or symptoms of an illness, within 48 hours prior to AWANA, ask that you keep your child home and DO NOT send them to AWANA.
In the event your child attends AWANA, and you, your child, or a family member is diagnosed with COVID-19 at any time during or with 14 days after attending AWANA, please let the Kersey Community Chruch know immediately so we can take necessary action.
Parental Consent
I hereby consent to let my child participate in the event AWANA. It is understood that every precaution will be taken for the safety and well-being of my child, but in the event of accident or sickness, Kersey Community Church, its staff and its volunteers are hereby released from any liability.
