POLICY AGREEMENT
GAVH reserves the right to treat your pet in the case of an emergency. GAVH prohibits leaving personal items with pets for safety and sanitation reasons. I authorize the exam and/or the administration of the tests and treatments selected above.

Please note that single night boarders may not have requests for photos, baths, or playtimes fullfilled depending on drop off and pick up times.
