    This is the form program directors should fill out if they wish to make any changes to their approved trip proposals. Changes may be made only once and should be submitted at least 12 weeks prior to program departure.

    Director of Learning Abroad and/or Department Chair and Academic Dean: Please sign and date the section below after reviewing the proposed program changes. Signing signifies approval of the changes. Please do not make any other changes to the form without first consulting with the listed Program Directors.
