I understand that filling out this form does not guarantee a tattoo appointment with requested artist, it is just a request form and it is up to chosen artists' discretion whether they would like to take on this piece or not.

I understand that in order to book an appointment, I must place a non-refundable deposit with the studio to secure an appointment slot that will come off of the final price of my tattoo. I acknowledge that deposits are non-transferable between clients and will be forfeit if I fail to show up for my appointment, arrive more than 15 minutes late* to my appointment or provide less than 48 hours notice when rescheduling/cancelling. I am aware my deposit will not be refunded if I choose to cancel my appointment.
