Lamonica’s collects and uses Personal Information for health and safety and business-related purposes and to be in legal compliance. Below are the categories of Personal Information we collect and the purposes for which we intend to use this information:

• Identifying information, such as your full name and signature.
• Contact information, such as the name of your employer and telephone numbers
• Surveillance information, such as video surveillance.
• Medical and Health Information, such as your body temperature, COVID-19 diagnosis and related testing results, whether you have or display certain symptoms such as fatigue, cough, aches and pains, runny or stuffy nose, sore throat, diarrhea, headaches, sudden loss of smell or taste, or shortness of breath, whether you have been in close contact in the last 14 days with anyone who has exhibited any of these symptoms, whether you have been in close contact in the last 14 days with anyone who has tested positive for COVID-19, and whether you have recently traveled out of the country.
• Information regarding friends, family, or other individuals with whom you have been in close contact within the past 14 days, such as medical and health information for friends, family or other individuals related to COVID-19 symptoms and their travel information.

The Company collects Personal Information to use or disclose as appropriate to:

• Comply with all applicable laws and regulations.
• Manage and monitor your access to company facilities, equipment, and systems.
• Administer and maintain the Company's operations, including for safety purposes.
• Exercise or defend the legal rights of the Company and its employees, affiliates, customers, contractors, and agents.
• Reduce the risk of COVID-19 in the workplace and to permit contact tracing.

Employee, Temporary Employee, Applicant, Independent Contractor and Visitor Privacy Policy

In recognition of our employees', temporary employees’, applicants’, independent contractors’ and visitors’ privacy rights, Lamonica’s has adopted these basic policies:

• Lamonica’s will limit the collection of employee, applicant, temporary employee, independent contractor and visitor information to that needed for business and legal purposes.
• Lamonica’s will protect the confidentiality of all personal information in employee, applicant, temporary employee, independent contractor and visitor records. It will refuse to release personal information to outside sources without an employee's, applicant’s, temporary employee’s, independent contractor’s or visitor’s written approval, unless legally required to do so.
• Lamonica’s will limit record access to staff members with an authorized, business-related "need-to-know" or to third parties who have obtained a court order or subpoena for specified employee records.
  Staff who handle personnel records must adhere to these basic policies or face disciplinary action.

Employees may review information contained in their personnel files upon request.

Employees also can correct inaccurate information or submit a written statement rebutting disputed documents contained in their files.
