Boarding Requirements:

BOARDING REQUIREMENTS: All Canine boarders must be current on the following vaccines, DHPP, RABIES, BORDETELLA, LEPTOSPIROSIS and INFLUENZA.All Indoor Only Feline Boarders must be current on the following vaccines, FVRCP.All Indoor/Outdoor Feline Boarders must be current on the following vaccines, FVRCP, FELV, and RABIES.All Feline and Canine Boarders must be current on a physical exam within the last year by a staff veterinarian of The Pet Doctors of Sherm Oaks.All Feline and Canine Boarders must be current on a monthly Prescription Flea Treatment such as Simparica or Revolution.PRIOR TO YOUR PETS BOARDING STAY, All Feline and Canine Boarders must have a current NEGATIVE FECAL TEST on file within the last year of thier boarding stay.

Feedings:

We feed Purina EN Dry food for dogs and Hills Science Diet Dry food for cats. However, you may bring your own food if your pet is on a special diet, or (if you prefer to feed them their regular diet). If you do not bring your own food and we carry the special diet you will be charged for the special diet and any remaining food will be sent home with your pet(s).

Personal Items:

Leave personal items here at your own risk (such as dog beds, toys, blankets). I understand that The Pet Doctors of Sherman Oaks is not responsible for any damaged or lost articles that I may bring with my Pet(s).

Boarding Classifications:

You will be given an estimate for your pets boarding stay. Depending on your pet’s needs, the fees for boarding will vary. Regular Boarding is for Pets over 4 months of age that are not on any medication. Medical Boarding applies when any type of medication services are provided or needed for your pet(s) during their stay. Puppy Boarding is for dogs up to 4 months of age.Medical Boarders, IF YOU DO NOT BRING YOUR OWN MEDICATION, YOU WILL BE CHARGED FOR MEDICATION(S) USED WHILE YOUR PET IS IN OUR CARE. Plus any additional services that may be necessary i.e. examinations, lab testing, and/or treatments as deemed necessary by the doctor.

Diabetic Boarding:

Diabetic dogs and cats require a higher level of care and observation, and will be cared for as hospitalized pets by the medical team. If needed, a customized treatment plan and estimate of cost will be provided for the stay.

Check Out Time:

Your Pet(s) must be picked up by before 12:00 noon in order to avoid an afternoon check-out fee. **The afternoon check out fee will be waived if your pet receives a bath on the day of check-out.**Your Pet(s) must be picked up no later than 5:30pm. AFTER HOURS PICK-UPS ARE NOT GUARANTEED AND MAY NOT ALWAYS BE AVAILABLE RESULTING IN AN ADDITIONAL BOARDING FEE. IF AFTER HOURS PICK UP IS AVAILABLE AND AGREED TO, THERE IS AN AFTER HOURS PICK-UP FEE THAT WILL APPLY AND IS CHARGED IN 30 MINUTE INCREMENTS.

Boarding Consent Form Duration:

This form will be kept on file and remain in effect for one year after signing or sooner as required due to updates to our policies.

During your pet's stay please know that our team will make every reasonable attempt to reach you and keep you informed should the need for additional and unexpected treatment arise.

In the event I can't be reached, by signing below. I hereby authorize The Pet Doctors of Sherman Oaks to perform any vaccination and parasite control treatments, and if necessary medical & or surgical procedures in order to ensure the health and wellbeing of my pet(s) and other pets within the hospital. Should my pet become soiled during their stay, I authorize The Pet Doctors of Sherman Oaks to bathe my pet(s) and charge my account accordingly.
