This application and all supporting documentation must be completed and submitted in its entirety in order to be considered for funding. You may be contacted by The Lellow Foundation if additional information is needed. Incomplete applications may be rejected.
To qualify for funding: The applicant must be a Special Education teacher currently employed at a Title 1 school in the greater Houston area. The beneficiairies of resources or materials funded by The Lellow Foundation must be children who qualify for special education services at the requesting Title 1 school. Minimum funding requirement is $100. Maximum funding limit is $1,000 per teacher, per academic year.
Following review and approval of your application, The Lellow Foundation will contact you directly with a purchase order form to complete your request. The purchase order will require signature approval from your current Principal and must be returned to The Lellow Foundation within 30 days of approval or funding may be revoked.
Submissions will be reviewed in accordance with approval criteria established by The Lellow Foundation Board of Directors. Please allow 60 days for review of applications.
