You enter upon the premises of the Tennis Academy of Guam at your own risk and are advised to take such precautions as necessary for your safety as you deem appropriate and as permitted by the Tennis Academy of Guam.

Acceptance of my entry form in these events is without assumption of responsibility of any kind by the Tennis Academy of Guam (TAG), Sheraton Laguna Guam Resort & Spa (Sheraton), Hilton Guam Resort & Spa (Hilton), Rick Ninete Tennis Courts (RNTC), Hyatt Regency Guam (Hyatt), Aurora Hotel, ASC Trust, Calvo’s SelectCare (CSC) and other sponsors, the host club owners, trustees or governing boards of facilities, committees, or the management of any event in which I may be entered of may participate. In consideration of the acceptance of my entry, I do hereby for and behalf of myself and my heirs and legal representatives release and forever discharge TAG, TAG instructors and coaches, Sheraton, Hilton, Hyatt, Aurora, CSC, ASC Trust and other sponsors, and host from any and all claims and demands of every kind, nature and character excepting gross negligence, reckless conduct, or willful, wanton conduct, which I may have or may hereafter acquire for any and all damages, losses or injuries which may be secured or sustained by me in connection with my activities during the period for which such permission is granted and any period traveling to or from such event described, and all claims are hereby waived and released and I covenant not to sue therefore, I understand that this tournament will be governed by applicable USTA rules and regulations, the rules and and regulations of this tournament, the rules and regulations governing the discipline of players in USTA / HAWAII Pacific Section sanctioned tournaments, the USTA Code of Conduct, tournament policy, and ranking regulations, and agreed to conduct myself accordingly. Further, in case of medical emergency, and I can not be contacted, I hereby authorize personnel associated with TAG to render first aid and/ or transport me/my child to a hospital or emergency medical facility for treatment. Additionally, I grant TAG, its clubs, representatives, officials, coaches, agents, employees, officers, successors, sponsors, to insert pictures or video of myself, my child/ren, or ward on our website and other promotional materials not limited to any form of media related to the sport of tennis. These photos or videos may be images of my child/ren, ward, or myself without expectation of payment.
