    Before proceeding, you are hereby informed that as a client, you are required to provide information which will be used as bases for the provision of the Museum's Biological Identification Services. The information which you will provide will only be used by the Museum to process your request and/or contact you for any concern. Your private information will not be divulged to anyone or any party. All information collected through this online form is strictly confidential and will be deleted/destroyed every six months.

    Due to the ongoing CoVID-19 pandemic and the imposed community quarantine measures, this communication for this service will be done online. Your submission will be forwarded to our respective section staff who will endorse your request to the Director. Once approved, the section staff shall contact you for instructions on how to send your specimens.

    Although our normal turnaround time is estimated to be around 7 government working days (for single specimens), please be considerate and allow us more time to identify a larger number of specimens due to the current situation.
