Animals are to be fully vaccinated with proof of vaccination if they were not given at LVH. Proof of vacination documents may be brought in the day of boarding or faxed to us as 406-222-4407 by you or your veterinarian. 

REQUIREMENTS FOR BOARDING

1. All animals must be current on all vaccinations.
2. All animals must be free of external parasites (e.g. , ticks, fleas, etc.), or they will be treated at owner’s expense.
3. Livingston Veterinary Hospital has my permission to do whatever is necessary should an emergency arise or sickness develop and we are unable to contact you during the boarding period .
4. If a tranquilizer is necessary for treatment or handling, Livingston Veterinary Hospital has my permission to administer such medication.
5. Pets may be picked up after 10:00 a.m. Monday through Friday.
