Take 5 Bodywork Covid-19 Waiver

Thank you for taking a moment to complete the Take 5 Bodywork Covid-19 waiver. This MUST be done prior to your session with Take 5 Bodywork. Please carefully review each statement, check the box to the right of each statement, and then sign at the bottom.

Even when no symptoms are present, Covid-19 can be a serious, highly contagious, and deadly disease.
Massage/bodywork is not appropriate care for infectious or contagious illness.
Even though massage and bodywork are permitted, there may still be risks.
If I contract Covid-19, even if I am not aware of symptoms, I may be at greater risk for blood clotting, deep vein thrombosis (DVT), pulmonary embolism, stroke and other serious life-threatening conditions, and even death. I further understand that massage is highly contraindicated in this case and can increase these risks.
I must immediately inform Take 5 Bodywork and cancel my appointment if I become aware of an infectious or contagious condition, am coming down with or getting over a "bug" (even if I don't think it's Covid-19).
The therapist/practitioner has the right to cancel/postpone my appointment if there is any doubt or concern related to my or her/his health.
I must properly wear a mask inside the massage therapist's office at all times and will continue to wear my mask throughout the entire duration of the session.
Take 5 Bodywork is unable to extend beyond the slated appointment time to accommodate late arrivals, so I'll do my best to be on time for my appointment.
Take 5 Bodywork may be obligated to report any known cases of Covid-19 to public health officials.
Even if I have already had Covid-19, I can be reinfected with the virus, and that symptoms in this case are often worse.
Even if I have been vaccinated, it is not a guarantee that my community and I are completely protected from Covid-19.

By signing below and accepting my appointments, I certify that I understand the above and will adhere to new Covid-19 protocols and guidelines to the best of my ability, including immediately informing Take 5 Bodywork if I suspect that my health may, in any way, be jeopardized. I understand that, even though massage and bodywork may currently be permitted by public health officials, there are still risks involved with receiving treatment and, moreover, that this permission and privilege can also be rescinded at any time should officials or Take 5 Bodywork become concerned over increased risks. I will do my best to keep our entire community safe. I indemnify and hold harmless Take 5 Bodywork (including its owner, staff, practitioners, landlord and property management team of its premises,…) on any account.*
