This form is required for any Captains candidates seeking points for specific assignments in the resume portion of the process. Please complete one form for each assignment for which you seek credit. Candidates may complete more than one form per process. Submitting this form does not guarantee credit will be granted per the request. To obtain credit, the supervisor/chairperson identified must validate the information you provide. In order for the Resume Panel to consider granting credit for your assignments, completed validation form(s) must be recorded at the time of submission in the promotional process.
