At Horseshoe Resort, we want to ensure the Health and Safety of all our Guests and Staff. Prior to coming to the Resort, we ask that you complete the following COVID-19 self-assessment.
Reason for visiting the Resort:
Do you have any of the following new or worsening symptoms or signs? Symptoms should not be chronic or related to other known causes or conditions.

Runny/stuffy nose or nasal congestion
Decrease or loss of smell or taste
Nausea, vomiting, diarrhea, abdominal pain
Not feeling well, extreme tiredness, sore muscles
Have you travelled outside of Canada or had close contact with anyone that has travelled outside of Canada in the past 14 days?
Have you been in close contact with someone who is sick or has confirmed COVID-19 in the past 14 days?
