I hereby authorise you to release any information relating to the Workers Compensation claims to Actevate and engage them in accordance with their accreditation as a Rehabilitation Provider-SIRA Approval number 514.

We respectfully request that current and future workplace injuries where lost time is a feature of the claimant’s incapacity 7 days after lodging their injury notification are referred immediately to Actevate.
