"Remarks: As per MARPOL at least 25% of the Cargo tanks must be Crude Oil washed for Sludge control. This must include the Heavy weather ballast tank which must be full-washed."
Remarks:Ensure MSDS is available for Cargo carried on board, Paints, Chemicals carried on board, etc. and ensure they are all displayed in their stowage positions. The sample locker must have MSDS for cargo/bunker samples.
Remarks:Is each hose must be marked with : Max working pressure/ temperature Test date and Serial number Date of manufacture The name of each product for which the hose may be used: for Oil products ”OIL SERVICE” or for Hazardous Material-“HAZMAT SERVICE SEE LIST” followed by a letter, number or symbol that corresponds to a list on board which identifies the products that may be transferred through that hose as per CFR 154.5.
Remarks: Often vessel's cranes may not be certified specifically for personnel transfer.
If so, ensure all precautions and checklists complied with as per company procedures before using the crane for the transfer of personnel. Confirm the company procedures regarding the same are being complied with.

Following STS documentation to be checked for compliance:-1. STS Checklists as per latest ICS/OCIMF/SIGTTO/CDI guidelines edition 20132. The JPO (Joint Plan of operations) as provided by the service provider3. Risk assessment as submitted by the Service Provider4. Detailed Mooring Plan of participating vessels.5. Copies of certificates of fender and hoses6. Notification to coastal authorities7. Details of Drills associated with the specific STS Operation8. Records of Crew Experience Post feedback/ assessment by the Master.The vessel must request for evidence of suitable qualifications of the POAC before any STS operations.

Incineration:
Time & position of start & stop for incineration is to be recorded. Garbage Incineration is prohibited in the Baltic Sea-(Refer Helsinki Agreement 1990). There are restrictions in the Mediterranean sea (Refer Barcelona Convention). Check local regulations when in port (It may be prohibited even at anchorage for certain ports).
Grinding and Comminuting: Comminuter must be sealed before arrival port when less than 3 miles from land (Expected to be sealed during the inspection.) Comminuter must be certified to reduce food size to 25mm or less.
For Tankers: Confirm quick sampling ports are available on Ballast tanks.In case the forepeak tank is completely separated from cargo tanks without any common bulkhead, there is no need to check Ballast unless there are hydraulic lines passing through it.If it is required to enter a void space in order to sample the water in the Forepeak tank, this will be an observation.If ballast tank domes have butterfly nuts, separate Quick sampling ports are not needed.
As per VIQ: For ship staff, the use of LSA FFA must be given not later than 2 weeks of joining. Detailed training on LSA FFA to be done not later than 2 months which must be repeated every 2M. (This is not familiarisation, it must be carried out for all crew every 2 months in rotation) For visitors who are on the ship for a short period, basic familiarisation to be completed not later than 24 hours of joining.

Note: Tankers above 20000 T DWT built after 1st Jan 2012 must-have fixed Gas detectors in all ballast tanks and void spaces including Forepeak tank and other spaces next to Cargo tanks.
Note:
Mooring Line design brake force = 100 - 105% of Ship Design MBL / Tails to have the same rotation properties as the mainline; Synthetic tails to have TDBF = 25-30% higher than Ship Design MBL(SDMBL). Length of mooring tails mentioned in and as required by the mooring arrangement plan. Tails to be retired before they reach 75% of the SDMBL.
It is surprising to note so many instances where checklists are ticked off even though the specific check or equipment is not present on board. Take time to go through and ensure all ticks are applicable.

These values of TLV-TWA of gas and OEL and actual gas reading must be clearly mentioned in the permit. The operator may have lower value requirements. If in any emergency, space that is not cleared has to be entered, the operator permission is required.
