Playtime Waiver if Selecting Group Play Sessions I understand that my dog’s attendance into doggie daycare playtime at Morning Star Farm Pet Resort, LLC is not without risk to my dog, because some of the dogs to which the pet may be exposed may be difficult to control and may cause injury even when preventative measures are taken. I understand doggie daycare involves contact with other dogs in a group environment and is not constantly supervised by Morning Star Farm Pet Resort, LLC, its members or employees. I hereby waive and release Morning Star Farm Pet Resort, LLC and its members and employees from any and all liability of any nature, for injury and damage which I or my dog may suffer, including specifically, but not without limitation, any injury or damage resulting from the action of any dog, including my own and I expressly assume the risk of any such damage or injury while attending doggie daycare at Morning Star Farm Pet Resort, LLC or while on the grounds or the surrounding area thereto. I hereby agree to indemnify and hold harmless Morning Star Farm Pet Resort, LLC, its members, employees and guests from any and all claims, or claims by any member of my family or any guest that may accompany me to the facility, as a result of any action by any dog, including my own. I understand I am to use appropriate measures and preventative to keep my pet parasite free (fleas, ticks and worms) and that Morning Star Farm Pet Resort, LLC is not responsible if my dog contracts a parasite or virus from any other dog. Morning Star Farm Pet Resort, LLC also reserves the right to remove any dog from play sessions if parasites or aggression present.
Morning Star Farm Pet Resort, LLC Client Agreement Our establishment agrees to exercise due and reasonable care and to keep the premises sanitary and properly enclosed. Your pet will be fed, watered regularly and housed in safe, clean quarters. Rates are charged daily Not nightly. There is a 2 day minimum stay. There is also a minimum stay for holidays. Health and Immunization records must be presented at time of admission. Dogs must be immunized for DA2PP, Rabies & Bordatella is recommended. Cats must be immunized for FVRCP & Rabies. It is the responsibility of the owner to keep all pets current on vaccinations. It is the owner’s responsibility to keep all pets free of any internal or external parasites and authorize Morning Star Farm Pet Resort, LLC to treat for such ailments if they are presented during my pet’s care. All necessary parasite treatments will be charged in addition to the daily rates for the care of pets. Should any pet become ill or need medical attention, Morning Star Farm Pet Resort, LLC reserves the right to administer aid and to render care by your local designated veterinarian, if available, or by any other available veterinarian. Any expenses incurred, shall be paid promptly by the owner in addition to daily rates for the care of my pet. Any pet requiring visits to the veterinarian shall be charged an additional $50 fee. I agree to take full responsibility, financially or otherwise, for any property damage my pet causes to Morning Star Farm Pet Resort, LLC, or for any illness or injury to my pet which is beyond the control of Morning Star Farm Pet Resort, LLC, its members or employees. Owners may provide hard toys (no cloth at this time) for pets however Morning Star Farm Pet Resort, LLC will not be responsible for any loss or damage to such items. All food, treats, etc must be provided in plastic containers with pet’s name, last name and ration clearly marked. Additional charges may apply if preparation exceeds normal expectancy and will be at the discretion of Morning Star Farm Pet Resort, LLC. Morning Star Farm Pet Resort, LLC will happily provide medications provided by owner to your pet for ongoing conditions for an additional charge. The services we provide are done so without liability for loss or damage from disease, death, running away, theft or fire, and from injury or damage done by your pet to people, other animals or property, or other unavoidable causes. Due diligence and care has been exercised. All payment is due in full at the time services are rendered. The owner of this pet or his agent agrees to pay reasonable legal fees and costs incurred by the kennel in the collection of outstanding bills. No pet will be released until all charges are paid in full or other arrangements satisfactory to our kennel have been made. The customer agrees to notify us at check-in if there is any change in the pet’s pick up date or charges will be incurred for the full reservation. Any pet left uncalled for seven days from the scheduled date of pickup, as noted herein, shall become the property of Morning Star Farm Pet Resort, LLC and may be sold or disposed of. We accept Cash, VISA or MasterCard, American Express, and Discover Morning Star Farm Pet Resort, LLC does NOT accept pets that are aggressive to staff. There is a $35 returned check fee. Credit cards on file will be used in case of emergency veterinary treatment. We reserve the right to charge this card in the event of a delinquent balance due, or cancellation of reservation for boarding which do not meet our 7 day notice policy.
