In order to minimize the face to face contact during your appointment , please fill out the attached forms which will need to be signed and returned before the appointment. This ensures that there is no forms or signature required on the day of appointment.

Please come to your appointment alone unless you are bringing a parent or carer.
The front door will remain locked and you will need to phone the number above which will be answered by one of the staff who will come and open the door. This number should only be used for booked appointments.
All necessary belongings you have with you will be placed in a plastic box downstairs and not brought into the surgery.
A member of staff will greet you and take your temperature and will offer you hand sanitizer before entering the practice. This staff member will escort you straight up to the surgery.
Any patient who has symptoms or a high temperature will be asked not to enter the practice in order to minimize infection risk.
The dentist will be prepared for your treatment which you will have already discussed remotely.
The dentist and nurse will be wearing extra PPE.
Your treatment will then be provided for you. The dentist will complete as much as possible on that day in order to avoid the need for a further appointment too soon.
When your treatment is finished a member of staff will collect you and take you back to the front door.
