I acknowledge that there could be a risk of personal injury while in use of Vibrant Transportation premises.

I authorize, Vibrant Transportation to provide transportation services. I discharge, waive, and indemnify Vibrant Transportation, it's owner(s) and staff against all claims, injuries, property damage, accidents, and death.  Specifically including, but not limited to any negligence on the part of the owner(s) or it's employees.
