✨ Medical Guidelines
For safety reasons, we are unable to perform permanent makeup services if any of the following apply. If you have questions, please contact us before booking.

• Under 18 years of age
• Pregnant or nursing
• Taking blood thinning medications
• Active viral infections or uncontrolled medical conditions
• Diabetes without prior medical clearance
• Currently using Retin A or similar products
• Undergoing chemotherapy or radiation

Failure to disclose medical conditions may result in refusal of service without refund.

✨ Prior Permanent Makeup Disclosure
If you have had permanent makeup previously performed, prior approval is required before your appointment.

Failure to disclose previous work may result in service denial and a $200 cancellation fee.

✨ No Pets or Children Policy
For the safety of our clients and staff, no pets or children are permitted at appointments. Only the scheduled client may be present in the treatment area.

✨ Cancellation Policy
Your card will not be charged at the time of booking. It is securely stored solely for our cancellation policy.

• Same day cancellations or no shows will incur a $200 cancellation fee
• Arrivals 15 minutes or more past your scheduled time may result in refusal of service, and a cancellation fee may apply

Please read the following statements carefully:

Permanent makeup procedures including Microblading, Nano Brows, Ombre Brows, Eyeliner, Lip Blush, and Scalp Micropigmentation are cosmetic tattoo services.

Results typically last 12 to 18 months, though longevity varies based on body chemistry skin type lifestyle and aftercare. In some cases pigment may fade unevenly change tone or disappear entirely. Rarely pigment migration may occur.

I understand that discomfort swelling redness tenderness and minor bleeding are normal healing responses. Although uncommon, allergic reactions to pigments or topical anesthetics are possible.

I understand that improper pre care or failure to follow aftercare instructions may result in complications including infection patchy healing excessive fading or unsatisfactory results.

I acknowledge that if undergoing an MRI within three months of treatment I must notify my medical provider. I understand there is an inherent risk of scarring with any tattoo procedure and that healing responses vary by individual.

By signing below, I confirm that all information provided is accurate and complete. I understand the information above and consent to proceed.

Permanent makeup services will not be performed on individuals who:
• Are under 18 years of age

• Are under the influence of drugs or alcohol
• Are taking blood thinning medications
• Are undergoing chemotherapy or radiation
• Are currently using Retin A or similar products
• Have uncontrolled medical conditions or active infections

Medical clearance is required for clients with:

• Diabetes Type 1 or Type 2
• Autoimmune disorders
• Conditions that impair healing or increase infection risk

Failure to disclose medical information may result in refusal of service without refund.

By signing below, I confirm that I have read and understand the above information and consent to the use of topical anesthetic if deemed appropriate.

I understand that numbing effectiveness varies by individual skin type, sensitivity, and body chemistry. Complete numbness is not guaranteed, and some discomfort may be experienced during the procedure.

I understand that allergic reactions to anesthetics or numbing agents, while uncommon, are possible. Symptoms may include redness, swelling, rash, blistering, dryness, or irritation. If an allergic reaction occurs, I understand that I must seek medical attention and release the studio and artist from liability.

I understand that anesthetics are applied prior to tattooing and removed before the procedure begins. Temporary redness or swelling may occur and typically resolves within 1 to 4 days.

I confirm that I am over 18 years of age, not pregnant or nursing, and not under the influence of drugs or alcohol. The general nature of cosmetic tattooing and the specific procedure to be performed have been explained to me. 

I understand that permanent makeup is a tattoo procedure and an artistic service, not an exact science. Results vary based on body chemistry, skin type, lifestyle, medications, immune response, and aftercare compliance.

 I acknowledge that:
• Results typically last 12–18 months
• Pigment may fade, shift, soften, blur, or disappear entirely
• Oily or problematic skin may heal less crisp or appear more powdery
• Multiple sessions may be required to achieve desired results
• Final healed results cannot be evaluated until 4–6 weeks post procedure

 

I approve the shape, color, and placement prior to the procedure and understand that healed results may differ from the initial design. Dissatisfaction with healed results does not indicate error or negligence.

I understand that swelling, redness, tenderness, minor bleeding, or bruising are normal healing responses. Although rare, allergic reactions to pigments or anesthetics are possible. 

I understand that pigment may remain in the skin long-term even after fading and that removal may require medical or laser procedures which can cause scarring or permanent skin changes.

Although Rare, There is a possibility of an allergic reaction to numbing agent and/or pigments. By initialing below, I release the technician from liability if I develop an allergic reaction.

I agree to follow all pre-procedure and aftercare instructions. Failure to do so may affect healing and final results.

I understand that permanent makeup outcomes cannot be guaranteed and that there is no warranty regarding color retention, longevity, or final appearance.

I acknowledge that results may be affected by future skin treatments including injectables, lasers, chemical peels, plastic surgery, or other skin-altering procedures, and that some changes may not be correctable.

I acknowledge potential risks including infection, scarring, uneven color, pigment migration, or unsatisfactory aesthetic outcome.

I release Feminink Beauty Studio and Hudson Beauty Bar, its artists, employees, and representatives from all claims or liability related to the procedure, including outcomes related to body chemistry, healing response, or pigment behavior.

I certify that I have read and initialed the above paragraphs and have had explained to my understanding the consent and procedure permit. I accept full responsibility for the decision to have this cosmetic permanent pigmentation work done. I give Hudson Feminink Beauty permission to perform my Permanent Makeup procedure.

I understand that photographs and or videos taken before, during, and after my permanent makeup procedure are mandatory for medical, legal, quality control, and professional documentation purposes. These images are required to accurately assess skin condition, track healing progress, document results, and maintain proper client records.

I understand that refusal of photo documentation may result in refusal of service.

I acknowledge that all documentation images are securely stored and used internally for client records, education, and professional reference.

I understand that declining marketing or social media use does not affect my treatment and does not exempt me from mandatory documentation.

Permanent makeup is a cosmetic tattoo service and results are subjective and vary by individual.

All sales are final. Feminink Beauty Studio and Hudson Beauty Bar do not offer refunds or removal services under any circumstances due to dissatisfaction preference change healing outcome pigment retention color change or artistic opinion.

I understand that permanent makeup results are influenced by body chemistry skin type lifestyle medications immune response and aftercare compliance. The studio and artist are not responsible if color fades changes or does not retain as expected.

By proceeding I agree not to dispute or charge back this service.
