In signing this agreement, I agree that I am personally responsible for any damages whatsoever to the instrument while they are in my care, whether or not such damage is caused by me or from reasons beyond my control. I am also responsible for maintaining the instrument in a manner indicated by the Instrument Repair Technician. I agree that no one other than myself will use the instrument and that I will personally return the instrument to the Instrument Repair Office (Rm. CPFA 019) if requested to do so by ISU faculty or staff. It is also agreed that this instrument will be used only in preparation for and playing in ISU courses for which I am registered. If I drop a course, I must return the instrument within 48 hours. If I use the instrument for any reason not outlined above, I understand I forfeit my right to use an ISU instrument and will have the instrument revoked immediately.
If the instrument is damaged, I agree to pay for damages. If the instrument is damaged beyond reasonable repair, I agree to pay for a replacement instrument. The ISU School of Music prohibits the storage of instruments in automobiles for any length of time.
If the instrument is stolen, I will notify campus police and the ISU School of Music within 24 hours. I agree if the instrument is not recovered in a reasonable time frame, I will pay for a replacement instrument.
I agree to purchase necessary items to maintain the above instrument properly and in accordance with the guidelines set forth by the Instrument Repair Technician as outlined on the care card located in the instrument case. If I am no longer using an ISU instrument in the manner stated above, I will return the instrument to the Instrument Repair Office within 48 hours. If the instrument is returned dirty, I agree to pay a $60 cleaning fee.
All instruments must be returned by the date listed below with no exceptions. In the event that I do not return the instrument by the Return Date below, I understand that a hold will be placed on my ISU account. The hold will mean that I will be blocked from registering for classes, receiving a transcript or receiving a diploma. I understand that the hold will only be lifted once the instrument and/or accessories have been returned in satisfactory condition.
