By submitting this payment I understand and accept to receive communications from Midwest ADP via e-mail, text message, fax, phone and/or postal mail, and that these communications may include confidential information about my classes or SATOP programs. I further understand that any fees paid are refundable only prior to services being provided. If I schedule a class and do not attend without at least 48 hours written or verbal notice, I understand I may not be eligible for a refund, and may have to pay all or part of the class fee in order to schedule for a new date. If I attend class and am unsuccessfully discharged, I understand that I'm eligible only for a refund equal to the difference between amount paid and the value of services provided (SATOP CIP/SROP only). If I attend any other class and am unsuccessfully discharged during class, I understand I may not be eligible for a refund, and may have to pay all or part of the class fee in order to schedule for a new date.
