Our Medication Adjustment Service means treatment prescribed by our specialist will be monitored for 1 to 3 visits after the medication has been started. Our Medical Doctors on this service will collaborate with the specialist involved in your care to help find the right dosage for your medication, before your care is returned to your own doctor in the community.
AGREEMENT FOR ACCEPTING OUR REFERRAL AND MEDICATION ADJUSTMENT SERVICES
Our Medical Doctors are here to make initial adjustments to your medication plan. They will work with our specialists in Psychiatry to monitor the start of your treatment plan. Once this initial monitoring is complete over 1 to 3 appointments, your care will be returned to your own Family Physician in the community, or to your child’s Paediatrician. Our Medical Doctors on the Medication Adjustment Team at Possibilities do not provide general primary patient care or ongoing medication refills and support.
If you are enrolled in a Rostered Practice, please be aware that seeing another Family Doctor affects your primary care physician’s billing. Please inform us if you are a patient in a Rostered Practice. We can check with your primary care physician to determine whether using this service is an acceptable option to support your care.
Our Medical Doctors Services are offered only through secure video sessions and not in-person. Therefore, you will need to keep track of measurements like heart rate, height and weight at home. For blood pressure, you’ll need a home blood pressure monitor. Recommended BP monitors can be found at
. Cuff sizes on blood pressure monitors are different for different ages. So if this service is for your child, be sure to purchase the appropriate cuff size.
In some cases, our Medical Doctors and specialists will determine that it may be unsafe to prescribe medications without an in-person physical exam. Your own Family Physician will need to provide this service while collaborating with us since we cannot perform physical exams over video sessions.
