  • Contract for Incomplete

    This is an agreement between the faculty member and the student.
  • NOTICE

    Unless the extended access period for grade entry has passed (11:59 PM CST the second Tuesday after the end of term) Incompletes need to be assigned in MyPark Faculty Course Control. Submission of this form during the access period will not be accepted and the faculty will receive an email from the Registrar's Office with instructions to assign/update in MyPark.
  • Please Assign the "I"ncomplete in MyPark Faculty Course Control

    There is no need to complete the Contract for Incomplete form during the grading window.
  • After you assign the "I" in MyPark an email, triggered by the entry, will auto-send the following day to the faculty member and student as notification of entry.
  • The instructor needs to respond within 3 business days with the requirements and deadline.
  • FAST will extend Canvas access 2-3 business days after “I” is assigned
  • Please allow 5 business days after the incomplete form is submitted before the course is re-opened in Canvas. 
  • Final assessment of the grade is postponed to no later than the last day of the term/semester immediately following term/semester the course was taken.
  • Failure on the part of the student to complete the work will result in a grade of "F".
