    Note: Not all prompts will be present for each symptom listed. Use as a guide to access whether the participant experiences the specified ADHD symptom.
  • 1. Fails to give close attention to details or makes careless mistakes in schoolwork.
  • 2. Fidgets with hands or feet or squirms in seat.
  • 3. Has difficulty sustaining attention in tasks or play activities
  • 4. Leaves seat in classroom in other situations in which seating is expected.
  • 5. Does not seem to listen when spoken directly.
  • 6. Runs about or climbs excessively in situations in which it is inappropriate.
  • 7. Does your child not follow through on instructions and fails to finish work.
