Referral Program Terms of Use 1/1/2022

You are invited to be a member of our MedicarePrepare® Ambassador’s Club and earn up to $100 for each Medicare Advantage Plan enrollment as a direct result of referrals that are generated by your participation in our Referral Program.

We will provide you with Referral Cards with your unique Referral Code to pass out and distribute to your friends and family.

An experienced, licensed Agent will work with your Referral customers to guide them through the process of selecting and enrolling into a plan.

Referral Checks are issued to you on a periodic basis throughout the year. To be eligible to receive a Referral Fee, your referral prospect should call the Agent on the Referral Card and when prompted, give the Referral Code listed on the card.

The Referral Fees are based on the following products:

Medicare Advantage Plans:
● $100 New To Medicare & $50 for plan changes
● Long-term-care Insurance: $50
● Life Insurance Plans: $25

This is a one-time Referral Fee, per enrollment. A limit of one Referral Fee will be paid per family who enrolls with us.

Note: We are NOT able to compensate a Healthcare Provider ie: Pharmacies, Physicians, Medical Specialists, Clinics, Hospitals, Vision Care Providers, Dentists, Assisted Living Facilities, Chiropractor etc.

Spouses and family members living in the same household of our Agents are ineligible to participate in this program and receive a Referral Fee.

This offer is void in any state where it is prohibited by law. Program terms are subject to change without prior notice.

This agreement as of 1/1/2022 may be amended or discontinued at any time at the sole discretion of MedicarePrepare®
