Transcript Release Permission

This Transcript Release Permission form is for Emmaus students taking Pre-Professional Health Science courses or 3+1 courses at the University of Dubuque while enrolled at Emmaus Bible College.

I hereby authorize the University of Dubuque (UD) to share my official and/or unofficial transcripts with Emmaus Bible College (EBC) while I am enrolled in the Pre-professional Health Science concentration or a 3+1 major at Emmaus. UD transcripts will be released without charge upon request from Emmaus for the purpose of the Pre-Professoinal Health Sciences and 3+1 programs only. This agreement will expire once I have graduated from one of the above programs, or am no longer a student in one of the above programs, or upon my request to do so.

I also authorize Emmaus Bible College to release my official Emmaus Transcript to UD, free of charge, when I transfer to UD for the final year in the 3+1 program only, and I authorize UD to release my final official transcript, free of charge, to Emmaus upon completion of my 3+1 major at UD.
