  • Waxing and/or Sugaring Hair Removal

    Some information about you, and some information about these services
  • Waxing is the removal of hair using either hard or soft wax. The wax is typically applied in the direction of hair growth, and removed against the direction of hair growth. This results in the hair being removed from the follicle by the root. The wax can stick to the skin as well as the hair, and can cause redness, pain, and inflammation. If not done properly or unknown skin conditions are not disclosed, waxing could lead to painful or infected ingrown hairs, or bruising. When the client keeps the trained professional informed about their skin, the results are smooth skin that can last up to 6 weeks.

  • Sugaring hair removal, an ancient middle-eastern practice, is similar to waxing, but the paste sticks to the hair not to the skin, which causes less redness, pain, and inflammation. The paste is typically applied against the direction of hair growth, and removed with the direction of hair growth. This results in the hair being removed from the follicle by the root. Since the mixture is made of natural ingredients, there can be less irritation than waxing and it washes off easily with water. Just like waxing, if not done properly or unknown skin conditions are not disclosed, sugaring could lead to painful or infected ingrown hairs, or bruising. When the client keeps the trained professional informed about their skin, the results are smooth skin that can last up to 6 weeks.

  • Dermaplane or Chemical Peel Treatment

  • Dermaplaning is a form of manual exfoliation similar in theory to microdermabrasion but without the use of suction or abrasive crystals. An esthetician grade, sterile blade is stroked along the skin at an angle to gently “shave off” dead skin cells from the epidermis. Dermaplaning also temporarily removes the fine vellus hair of the face, leaving a very smooth surface
  • A chemical peel treatment is a form of chemical exfoliation which uses hydroxy acids to gently and effectively exfoliate dead skin cells and/or oils from the epidermis.

  • I consent to “before-and-after” photographs for the purpose of documentation, potential advertising, and promotional purposes.*
  • Reservation & Cancellation Policy for all current and future appointments: a valid credit card is required for all appointments. Please do not forget to confirm your appointment when you receive your reminder. In the event of cancellations received less than 24 hours prior to appointment a cancellation fee of 50% of the reserved service booking will incur. (Except in cases of contagious illness as described below.) No Shows will be charged 100%*

In addition to long-held and explicit sanitation measures this business has always adhered to, new preventative measures have been put in place to further reduce the spread of this novel coronavirus. However, these best practices still offer no guarantee regarding your potential risk of being infected.

  • I have read the above information. If I have any concerns, I will address these with my skin care therapist. I give permission to my therapist to perform the procedure(s) we have discussed, and will hold him/her and his/her staff harmless from any liability that may result from this treatment. I have accurately answered the questions above, including all known allergies, prescription drugs, or products I am currently ingesting or using topically. I understand my esthetician will take every precaution to minimize or eliminate negative reactions as much as possible. In the event I may have additional questions or concerns regarding my treatment, I will consult the esthetician immediately. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures.

    I certify that I have read, and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold the esthetician, whose signature appears below, responsible for any of my conditions that were present, but not disclosed at the time of this skin care procedure, which may be affected by the treatment performed on the date of service.

    I understand that no specific guarantees of the results can or have been made and that there is the possibility I may require additional treatments/procedures to obtain the expected results at an additional cost. I have read and understand all pre-treatment, post-treatment, and home care instructions. I understand the importance of following all instructions given to me. In the event that I have additional questions or concerns regarding my treatment or post-treatment care, I will consult the technician/esthetician immediately.
